Appointments:
Appointments are scheduled Monday through Friday, usually between 8 a.m. and 4:30 p.m. Please arrive at least 30 minutes early if this is your first visit to us or if you have new insurance information to give us. We require 24-hours notice when you are unable to keep appointments. After hours, you can leave a message on our machine. We charge $50.00 for appointments that are not kept.

Telephone Calls:
We do not charge for telephone calls. In fact, we encourage communication if you have any questions about your treatment. However, please make non-emergency calls during office hours, so that your chart will be available for review.

We will try to return your call as soon as possible. If your call is after hours, our on-call physicians can be paged in an emergency. Simply dial our office number, follow instructions for paging and enter your area code and phone number. NO PRESCRIPTION REFILLS AFTER HOURS. If you have not heard from us within an hour, please try again and make sure you are entering your phone number correctly, including the area code.

Financial/Insurance:
Our charges reflect the customary fees of physicians in our community. They are based on the number and complexity of your problems. At the end of an appointment, you will receive an itemized charge slip that shows the services rendered, the fee, the diagnosis and the suggested time of your next visit. You may pay by cash, check or credit card. We accept most major credit cards, including Visa, MasterCard, Discover and American Express. In addition, we will be happy to discuss financing options for cosmetic procedures. For cosmetic procedures, payment is due before treatment begins. For medical procedure, your co-pay is due at the time of service.

To help us keep accurate records, please let us know of any name, address, phone number or insurance coverage changes. Before your appointment, please discuss your insurance coverage with us. If we are contracted with your insurance company, we will handle your claim according to our agreement with them.

As a courtesy to our Medicare patients, we will be happy to bill Medicare for you. However, you are responsible for the 20% co-payment and any remaining deductible. If you have a secondary policy, we will be glad to bill that for you as well. Please check with your insurance company about your co-pay before your visit. If the co-payment is not made at the time of your visit, a $10 service fee will be added to cover the cost of invoicing you.

If an HMO insures you, you must provide us with a referral authorization at the time of your visit. Without an authorization, you will be responsible for all charges, as we have no way to invoice your insurance.


Prescription Refills:
If you need a refill, please call your pharmacy, not our office. The pharmacy will then notify us and we will respond within 48 hours. Call a few days before your prescription is finished. FOR SAFETY REASONS REFILLS CANNOT BE HANDLED AFTER HOURS AND ON WEEKENDS, SINCE THE ON CALL PHYSICIAN MAY NOT HAVE ACCESS TO YOUR RECORDS.
